Initial Contact & Assessment
QBE Case Worker, Matt collects information about a Jill’s injury to identify her needs and develop a plan.
Strategy & Implimentation
Matt sets specific objectives, goals and actions to meet Jill's identified needs, in collaboration with stakeholders such as her Dr, he puts the plan into action, organising, securing, integrating and working in partnership with those involved to achieve the planned goals.
Monitor Treatment & Peer Review
Matt checks Jill's progress and identifies any barriers or problems early.
Recovery & Evaluation
Matt measures the quality of Jill's outcomes and determines whether the action and plan has produced the desired results.
